Suffolk Disability & Health Action Group has been working with disabled people to produce ‘About Me’ (previously described as a Health Passport), a document that disabled people and/or their family carers can complete to give health and care professionals information about themselves to help those professionals provide better care.
This initiative has been welcomed both by disabled people and the organisations that work with them.
How to obtain ‘About Me’
‘About Me’ will be made available through a link published on a number of websites in both the statutory and voluntary sectors so it can be downloaded and completed by the disabled person and/or their family carer.
Unfortunately, no funding is available for printing hard copies of ‘About Me’.

Health service endorsement being sought
Suffolk Disability & Health Action Group is currently seeking endorsement of the use of the ‘About Me’ initiative from the Clinical Commissioning Groups in Suffolk, Acute and Community Hospitals, Community Healthcare Services and GPs in Suffolk.
This is important as some disabled people have told us that currently health professionals do not always read or take note of their current ‘Health Passport’ type documents resulting in the frustration of having to repeat their needs over and over again.
How to use ‘About Me’
‘About Me’ documents will be owned and kept up to date by disabled people and/or their family carers and they can be shared with many health and care professionals including Consultants, GPs, nurses, dentists, and paramedics and homecarers.
